1. Acceptance of Terms By accessing or using the services of Harmony Counseling & Wellness PC (“we,” “our,” or “us”), you agree to comply with and be bound by these Terms and Conditions. Services Provided Harmony Counseling & Wellness PC provides mental health counseling, therapy, and wellness services. Our services are intended for individuals seeking professional mental and emotional well-being support. Licensed professionals provide services per applicable state and federal laws. We do not offer emergency services. If you are experiencing a medical or psychiatric emergency, please call 911 or visit the nearest emergency room. 3. Client Responsibilities By engaging in our services, you agree to: Provide accurate and complete information about your health and medical history. ​Attend scheduled appointments or provide timely notice for cancellations. Follow the agreed-upon treatment plan and recommendations made by your therapist. Treat all staff and providers with respect and professionalism. 4. Payment and Fees Fees for services will be discussed during the initial intake session and are subject to change with notice. Payment is due at the time of service unless prior arrangements have been made. We accept insurance plans and self-pay clients; however, it is the client’s responsibility to verify insurance coverage and benefits. A cancellation policy may apply; missed appointments or late cancellations may result in a fee. 5. Confidentiality and Privacy We are committed to protecting your privacy in compliance with HIPAA and other applicable laws. Your information will be kept confidential except in cases where disclosure is required by law, such as: Risk of harm to yourself or others. Suspected abuse or neglect of a child, elderly person, or vulnerable adult. Court orders or legal requirements. For more details, please refer to our Privacy Policy. 6. Use of Website and Digital Communication Our website and online communications are for informational and scheduling purposes only. While we take reasonable measures to secure digital communications, we cannot guarantee the security of electronic transmissions. By using telehealth or email communication, you acknowledge the inherent risks associated with digital platforms. 7. Cancellation and Refund Policy Clients are expected to provide at least 24-hour notice for appointment cancellations. Missed appointments or late cancellations may incur a fee. Refunds for prepaid services are subject to review and approval by our administration. 8. Limitation of Liability Harmony Counseling & Wellness PC and its providers are not liable for any damages from using our services, website, or telehealth platforms. We do not guarantee specific outcomes from therapy or treatment. 9. Changes to Terms and Conditions We reserve the right to modify these Terms and Conditions at any time. Updates will be posted on our website, and continued use of our services constitutes acceptance of the revised terms. 10.
